29 CFR 1918.65(f)(1): Mechanically powered vehicles were not maintained in safe working order. a) M/V ARC-Integrity, Deck 4 - An employee was observed operating a Toyota 2.5 Ton Forklift (model 7FDU30, serial number 62223) while stowing household good crates in conjunction with other forklifts and the horn did not work. b) M/V ARC-Integrity, Main Deck - An employee was observed operating a Hyster 90 Forklift (model H90FT, serial number P005V04311J) while stowing household good crates in conjunction with other forklifts, and the rear view mirror was loose, and un-usable.
