29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items of this section: (a) The energy control (lockout) procedures for the Extruder #10, did not clearly state the different ways to isolate the machine and different sections that might be worked on, and where the main lockable disconnect switch is located for isolating the entire electrical power to the machine.

29 CFR 1910.147(d)(4)(ii): Lockout devices utilized were not affixed in a manner that held the energy isolating device in a safe or off position: (a) The lockout hasp and lockout lock for a maintenance employee working on the #10 Extruder did not hold the main power disconnect switch in the safe or off position in that the tab on the disconnect switch handle that the hasp went through was broken off and would not hold the disconnect in the safe or off position.
