29 CFR 1910.23(d)(1):     Every flight of stairs having four or more risers was not equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails:    (a) Billings, MT, Metra Park:  On August 7, 2012 and at times prior thereto, employees were exposed to falls of approximately 4 feet while using metal stairs without handrails or starirails while entering and exiting trailers such as but not limited to the maintenance trailer for assembling, disassembling, and performing maintenance on carnival rides.

(c) Montana Fair, Billings, MT, Metra Park:  On August 7, 2012 employees exposed to falls above 6 feet to the ground level below while assembling, disassembling, and performing maintenance on the Thunderbolt without personal fall protection.
