29 CFR 1926.451(h)(1):   Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.    a) Scaffold Area - On or about August 10, 2012, the employer did not ensure toe boards or other forms of falling debris protection was provided to protect workers below or near the tubular weld scaffold system, thus exposing employees to struck-by hazards.

Crossbraces were used as a means of access.    a)  Scaffold system  - On or about August 10, 2012,  employees were using cross bracing and scaffold up-rights  to ascend and descend the second level scaffold system, thus exposing employees to fall hazards up to 14 feet.
