29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems: (a) Residential home site: On or about August 8, 2012 and at times prior thereto, employees were exposed to a fall of approximately 16 1/2 feet to the concrete driveway below while installing new roofing materials on a 9:12 pitch roof.

29 CFR 1926.503(a)(1): The employer did not provide an effective training program for each employee who was exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: (a) Residential home site: On or about August 8, 2012 and at times prior thereto, employees were exposed to a fall of approximately 16 1/2 feet to the concrete driveway below due to lack of adequate training in the recognition of fall hazards and the means to control these hazards.
