29 CFR 1926.1053(b)(1): Where a portable ladders was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a ridgid support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder.      Location: Jobsite, 256 Montoya Drive, Building 26, Units #50-#52, Branford, CT 06405      A) The base section of the Werner 24 foot Type I, model D1324-2, aluminum extension ladder did not extend at least three feet above the eave of the roof or upper landing surface of the roof.    B) The fly section of the Werner 24 foot Type I, model D1324-2, aluminum extension ladder did not extend at least three feet above the eave of the roof or upper landing surface of the roof.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:      Location: 256 Montoya Drive, Building #26, Units #50-#52, Branford, CT 06405      The Werner 24 foot, type I, model D1324-2, aluminum extension ladder's base and fly sections were used separately to gain access to the 10 in 12 pitched residential roof with an 8 foot 10 inch eave to ground fall distance.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502.    Location: 256 Montoya Drive, Building #26, Units #50-#52, Branford, CT    Each employee working on the 10 in 12 pitched residential roof with an 8 foot 10 inch eave to ground fall distance from the main roof and a 17 foot fall from the elevated dormer roof to the ground were not protected from falling by the use of a fall protection system.    REPEAT STATEMENT    Superior Remodeling LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 315809178, Citation number 01, item number 001a, issued on October 18, 2011, with respect to a workplace located at 36 Center Street, Prospect, CT 06712 and became a final order on November 14, 2011.
