29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a)  Ramos, Jose Daniel at 7311 and 7312 20th St. Rd., Greeley, CO 80634:  On or before August 11, 2012, the employer did not ensure that employees wore safety glasses when using or working in the vicinity of pneumatic nail guns.  Condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13):  Employees engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph (b).  Note:  If using these systems was infeasible or created a greater hazard, the employer did not develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502:    a)  Ramos, Jose Daniel at 7311 and 7312 20th St. Rd., Greeley, CO 80634:  On or before August 11, 2012, the employer did not ensure that employees used fall protection when shingling these two single family homes with roof pitches ranging from 4 in 12 and up to 6 in 12.  Conditions exposed employees to fall hazards of up to 16 feet.      Ramos, Jose Daniel was previously cited for a violation of this occupational safety and health standard or its equivalent, 29 CFR 501(b)(13), which was contained in OSHA inspection number 313412504, citation number 1, item 2, issued August 27, 2009 and became a final order on October 22, 2010, with respect to a workplace located at 2247 Mandarin Ct., Johnstown, CO 80534.

29 CFR 1926.502(d)(19):  Personal fall arrest systems and components subjected to impact loading were not immediately removed from service and/or were reused before they were inspected by a competent person and determined to be suitable for reuse:    a)  Ramos, Jose Daniel at 7311 and 7312 20th St. Rd., Greeley, CO 80634:  On or before August 11, 2012, the employer did not ensure that a personal fall arrest system lanyard was removed from service after it had deployed.  Condition exposed employees to fall hazards of up to 16 feet.
