The exposure control plan was not reviewed annually, including those requirements as listed at 1910.1030(c)(1)(iv)(A) and (B); there was no documentation at the time of the inspection that an annual review was made.  (Note: Five exposure incidents had occurred at the facility since 2008, with the last in 2011.)    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. 29 CFR 1910.1030(f)(1)(ii)(D):     The employer did not ensure that all medical evaluations or procedures including the hepatitis B vaccine, vaccination series, post-exposure evaluation, or follow-up, including prophylaxis, were provided according to recommendations of the US. Public Health Service current at the time these evaluations and procedure took place:    a. Post vaccination antibody (titer) testing was not provided to those employees who have blood or patient contact and are at ongoing risk for injuries with sharp instruments or needlesticks; at the time of the inspection OSHA was told that antibody testing had not been provided and no documentation was provided to show such testing was provided.  (Note: the 12-26-97 CDC guidelines indicate postvaccination antibody testing.)    Abatement certification and abatement documentation is required for this violation. 29 CFR 1910.1030(c)(1)(vi):     The Exposure Control Plan was not made available to the Assistant Secreatry and the Director upon request for examination and copying:    a. During the inspection, the employer had difficulty determing what was the Exposure Control Plan, locating the plan, and retrieving requirements of the plan, including, but not limited to, the Sharps Injury Log, and the employee exposure determination (during the inspection, when the exposure determination was unable to be located, the employer initially thought that all employees were considered to have occupational exlposure, however, an exposure determination that was later provided contradicted that understanding).    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.
