29 CFR 1926.20(b)(2): Programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer: Jobsite: 2nd floor outside room 210. A competent person had not inspected and secured a work area where a worker from a subcontractor was seriously injured when he contacted a newly installed lighting whip that was energized at 277 volts with exposed circuit conductors.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: Jobsite: 2nd floor outside room 210. An employee of a subcontractor was seriously injured while working in a ceiling cavity when he contacted the exposed conductors of a newly installed lighting whip that was energized at 277 volts.
