29 CFR 1910.23(e)(9):     Wall opening barriers (rails, rollers, picket fences, and half doors) shall be of such construction and mounting that, when in place at the opening, the barrier is capable of withstanding a load of at least 200 pounds applied in any direction (except upward) at any point on the top rail or corresponding member:     Employees were potentially exposed to serious injury from a fall to a lower level. The hazard was located in the Fire Cache building where the gate in the storage area was not secured by a positive means.

29 CFR 1910.132(d)(1):     The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):    (a) Employees were potentially exposed to serious injury from a fall to a lower level. The hazard was located in the Fire Cache, where the employer had not assessed whether fall protection was needed when employees were working next to a loading platform when the gate was opened.

29 CFR 1910.212(a)(3)(ii):     The  point of operation of machines whose operation exposed an employee to injury, was not guarded:    (a)  Fleet maintenance shop:  On or about August 7, 2012 and times prior thereto, the 50 ton hydraulic press was not guarded by a physical guard or by distance.  This exposed employees to serious injury if the work piece were to break and strike the employee.    (b)   Fleet maintenance shop:  On or about August 7, 2012 and times prior thereto, a drill press was not guarded at the point of operation.  This exposed employees to laceration hazard to the hands.

29 CFR 1910.305(b)(1)(ii):     Unused openings in cabinets, boxes, and fittings shall be effectively closed:    (a) Employees were potentially exposed to serious injury from a heater timer switch fitting with exposed electrical parts that was not effectively closed. The fitting was located by the north exit door of the Interagency Dispatch Center, at the Lakeview Fire Compound, address of 1000 S 9th Street in Lakeview, Oregon.

29 CFR 1910.178(a)(4):     Modifications and additions which affected capacity and safe operation were performed by the customer or user without manufacturers written approval, and the capacity, operation and maintenance instruction plates, tags or decals were not changed accordingly:    (a) The inspection revealed that modifications had been made to one of the forks on the Wiggins model #WG106ATS powered industrial truck, also known as "Mr. Wiggins", without the manufacturers written approval. Mr. Wiggins was located in front of the fire cache building at the Lakeview Fire Compound, address 1000 S 9th Street in Lakeview, Oregon.
