29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      a) On or about August 7, 2012, and at times prior, the employer did not ensure that employees were protected from amputation hazards while working with and near the following machines:    1) Ellis, mitre saw (serial # 16061735), saw did not have a guard on the saw blade.  2) Scotchman Punch Machine (serial # 1601k1-7) did not have a guard on the face of the puncher.  3) DeWalt, (hand grinder) did not have a guard on the cutting disk.
