29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. At the site, on the roof of a two-story, single-family residence under construction with an approximate 8:12 pitch, workers were performing roofing activities without any type of fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: At the site, on the roof of a two-story, single-family residence under construction with an approximate 8:12 pitch, workers were using a ladder to access and egress a roof to perform roofing activities which did not extend three feet above the landing surface, exposing workers to potential fall hazards.
