29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) Redwood Restoration Services, Inc. at 401 Park Ave., Eaton, CO 80615: On or before August 11, 2012, the controlling employer did not ensure that roofing subcontractor employees wore safety glasses when using or working in the vicinity of pneumatic nail guns. Condition exposed these employees to the hazard of projectiles to the eyes.

29 CFR 1926.502(d)(16)(ii): Personal fall arrest systems, when stopping a fall, did not limit the maximum arresting force on an employee to 1800 pounds when used with a body harness: a) Redwood Restoration Services, Inc. at 401 Park Ave., Eaton, CO 80615: On or before August 11, 2012, the controlling employer did not ensure that roofing subcontractor employees wore shock absorbing lanyards when shingling this church roof with a 9 in 12 pitch. Two of six fall systems did not have shock absorbing lanyards. Condition exposed employees to a fall hazards of 12'6".
