29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were  made without the manufacturer's written approval:    On August 14, 2012, at 200 Georgia, Deer Park, TX, employees were exposed to struck-by hazards when using a forklift with a 2740 pound load suspended underneath the load bearing tines while using only one tine, the method of "free rigging."

29 CFR 1926.758(f)(2): Both ends of all steel joists or cold-formed joists were not fully bolted and/or welded to the support structure before allowing an employee on the joists:    on August 14, 2012, at 200 Georgia, Deer Park, TX, employees were exposed to fall hazards at a height of approximately 16 feet when erecting a system engineered building.  The joist an employee was using as a working surface was fastened on one end to the column and the other end was supported by a forklift using the method of "free rigging."
