29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    a.  On or about August 14, 2012, an employee was required to wear a 3M 6000 half face respirators with 3M 6001 organic vapor cartridge while spray coating in the spray booth.  The employer had not established nor implemented a written respirator protection program.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:    a. On or about August 14, 2012, an employee wore SAS N95 Valved Particulate Respirators while sanding and grinding on sheet metals such as aluminum and stainless steel on a voluntary basis in the workshop. A medical evaluation was not conducted to determine if he was capable of wearing respiratory protection. The employer had not established nor implemented a written respirator protection program.
