1910.106(d)(5)(i): Combustible liquids shall not be stored so as to limit use of exits, stairways, or areas normally used for the safe egress of employees. a) At the maintenance shop - The employer did not store the combustible chemicals such as, but not limited to, Shell Antifreeze liquid and Milemaster HD-CJ-4-15W-40 Engine Oil properly and away from the exits doors and gates when employees were operating the Loomis trucks, exposing employees to the fire hazards.

29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii): a) At the maintenance shop -The employer did not implement and develop a written hazard communication program to employees working with the chemicals such as, but not limited to, Ultrapure Diesel Exhaust Fluid, 2942 CIA Degreaser, Shellzone Antifreeze/Coolant, and Shell Rotella T3 15W-40 which include ingredients like Ethylene Glycol and Potassium Hydroxide , exposing employees to eye, skin, and respiratory tract irritations.
