1910.106(d)(5)(i): Combustible liquids shall not be stored so as to limit use of exits, stairways, or areas normally used for the safe egress of employees.       a) At the maintenance shop - The employer did not store the combustible chemicals such as, but not limited to, Shell Antifreeze liquid and Milemaster HD-CJ-4-15W-40 Engine Oil properly and away from the exits doors and gates when employees were operating the Loomis trucks, exposing employees to the fire hazards.

29 CFR 1910.303(b)(1)(iii): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees.     a) At the maintenance shop - The employer spliced an extension cord to flexible cord of portable fan which left the insulation less than what the manufacturer originally used, exposing employees to electrical shock hazards.

29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):       a) At the maintenance shop -The employer did not implement and develop a written hazard communication program to employees working with the chemicals such as, but not limited to, Ultrapure Diesel Exhaust Fluid, 2942 CIA Degreaser, Shellzone Antifreeze/Coolant, and Shell Rotella T3 15W-40 which include ingredients like Ethylene Glycol and Potassium Hydroxide , exposing employees to eye, skin, and respiratory tract irritations.
