29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502.        (a) Jobsite, Top Roof:  On or about August 13, 2012, two employees were working from the top roof and they were not protected from falling.   The pitch of the roof was 9 in 12 and the fall hazard to the two employees was approxiamtely 19 feet.    (b) Jobsite, Back Porch Roof:  On or about August 13, 2012, an employee was working from the back porch roof and he was not protected from falling.  The pitch of  the roof was 6 in 12 and the fall hazard to the employee was approxiamtely 10 1/2 feet.

29 CFR 1926.451(g)(1)(i):     Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:        (a) Jobsite:  On or about August 13, 2012, an employee was not protected from falling while he was working from a ladder jack scaffold.  The fall hazard to the employee was approximately 16 feet to the ground.
