29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about 7/17/2012, employees were exposed to a fall from a height of approximately 20 ft. while working at the edge of a new construction building at 7210 Bandera Rd, San Antonio, TX.

29 CFR 1926.1053(b)(1): Where a portable ladder(s) was used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:    On or about 7/17/12, employees were exposed to a fall from height of approximately 20 ft, while using a fiberglass extension ladder to gain access to the roof of the commercial building at the Diamond Express Car Wash that did not extend 3ft. above the roof.
