29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about August 14, 2012 , and at times prior thereto, employees working on the residential property had not received training on the use of fall protection prior to being allowed to work in elevated places, exposing the employees to fall hazard of up to 20 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about August 14, 2012, and at times prior thereto, employees were required to use a 32 foot Louisville extension ladder to gain access to the rooftop and the ladders side rails were not extended at least 3 feet about the upper landing, exposing the employees to a fall hazard of approximately 20 feet.
