29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. Where roofing work was performed by four workers on a three story building at 736 S. Service Rd., Sullivan, MO on 8-14-12, with roof pitch that included 5:12, 7:12, and 8:12 sections, safety harnesses attached to an anchor via lanyards, or some other fall protection was not used. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii): a. Where roofing work was performed by four workers on a three story building at 736 S. Service Rd., Sullivan, MO on 8-14-12, with roof pitch that included 5:12, 7:12, and 8:12 sections, there was no formal training and documentation on the fall hazard requirements such as found at 1926.503(a)(2)(i) through (vii), including, but not limited to, when fall protection was required, at what roof pitch fall protection was required and options for fall protection, how to ensure anchoring personal fall arrest systems, and the requirements of the standard. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).
