29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    a)  On or about August 15, 2012 and times prior at 5460 Range line Rd, Mobile, Al:  The employer failed to ensure the top working level on a fabricated frame scaffold was fully planked or decked while performing bricklaying operations which exposed employees to hazards associated with falls of 25 feet 5 inches to the ground below.

29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied:    a)  On or about August 15, 2012 and times prior at 5460 Range line Rd, Mobile, Al:  The employer failed to ensure an employee did not use a fork lift with a pallet on the forks and raised up to the 3rd buck level to stand on while off-loading bricks from the pallet (platform) which exposed him to fall hazards of 15 feet 5 inches.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a)  On or about August 15, 2012 and times prior at 5460 Range line Rd, Mobile, Al:  The employer failed to ensure fall protection was provided for employees laying brick on the 5th buck level and an employee on the 4th buck level handing up brick which exposed employees to the hazards associated with falls of 25 feet 5 inches and 20 feet 5 inches to the ground below.
