29 CFR 1910.212(a)(5):     Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch:    a) In the pallet repair shop near the center workstation, on or about August 15, 2012 and at times prior to, the pedestal fan was not adequately guarded, in that the fan blade guard had a four-inch diameter opening, resulting in a struck-by hazard.    ABATEMENT CERTIFICATION IS REQUIRED.

29 CFR 1910.303(g)(2)(ii):     In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage:    a) At the electrical control box for the notcher, on or about August 15, 2012 and at times prior to, two sections of flexible conduit had separated from the box, which exposed the insulated electrical conductors that were supposed to be housed inside the conduit to physical damage, resulting in an electrical hazard.    ABATEMENT CERTIFICATION IS REQUIRED.

29 CFR 1910.95(g)(6):     At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels:    a) In the pallet repair shop, on or about August 15, 2012, annual audiometric testing was not provided for a nail gun operator who was exposed to noise at 132% of the action level (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of 87.0 dBA during the 346 minute sampling period, resulting in a noise hazard.  The exposure calculation included a zero increment for the 134 minutes not sampled.    b) In the pallet repair shop, on or about August 15, 2012, annual audiometric testing was not provided for a nail gun operator who was exposed to noise at 167% of the action level (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of 88.6 dBA during the 431 minute sampling period, resulting in a noise hazard.  The exposure calculation included a zero increment for the 49 minutes not sampled.
