29 CFR 1910.134(e)(6)(i): The employer did not obtain from the physician or other licensed health care professional (PLHCP) a written recommendation regarding the employee's ability to use the respirator: (a) Employees were using tight fitting 3M half mask air purifying respirators during concrete truck loading operations and the employer had not obtained a written determination from the physician conducting annual physicals as to whether the employees were able to wear the respirator as determined on August 16, 2012.
