29 CFR 1910.22(a)(1):     All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition.    a.     In the new room caustic pit west of the shaker, there were four handles which were used to raise grates which were approximatily 3 to 4 inches above the ground floor in the main walkway thereby exposing employees to a trip/fall hazard.    b.     In the closed head section of the main plant, the east trenches had grates over floor holes on the main walkway which were 3 to 3 1/2 inches above the floor surface thereby exposing employees to a trip/fall hazard.

In the closed head department, on the north side of the progressive flusher, the floor hole beside Tank Number 3 was 15 1/2 inches by 9 1/2 inches by 11 1/2 inches deep thereby exposing employees to a trip/fall hazard.

29 CFR 1910.23(c)(1):     Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder.     a.     On the south  side of the progressive flusher platform where employees process steel drums, there was no fall protection system such as, but not limited to, a guardrail thereby exposing employees to a 5 foot 3 inch fall hazard.    b.     On the north end of the chaining tank where maintenance employees were performing work, a fall protection system was not provided while on top of the chaining tank thereby exposing employees to a 7 foot fall hazard.    c.      On the loading dock at the rear of the building where employees unload steel drums to load onto the conveyor for recondiotioning, there was no fall protection system such as, but not limited to, a guardrail system on the dock thereby exposing employees to 4 foot 4 inch fall hazard.

29 CFR 1910.23(e)(1):     A standard railing shall consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level. The top rail shall be smooth-surfaced throughout the length of the railing. The intermediate rail shall be approximately halfway between the top rail and the floor, platform, runway, or ramp. The ends of the rails shall not overhang the terminal posts except where such overhang does not constitute a projection hazard.    a.    On the Closed Head label removing platform near the double end shimer, the top rail was installed at 33 inches and there was no midrail thereby exposing employees working on the platform to a fall hazard.    b.     On the south east end of the closed head label scraping platform, the top rail was installed at 33 inches and there was no midrail installed thereby exposing employees working on the platform to a fall hazard.

29 CFR 1910.151(c):     Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  [63 FR 33450, June 18, 1998]    a.     In the new room near the bung washer, the caps to the eye wash were missing, the eye wash needs a cover to prevent dirt and other contaminets into the eye wash in casen of an emergency thereby exposing employees to a contact hazard.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    a.     In the new oxidizer room on the Cutter platform where barrels which were closed head are cut and made into open head barrels, the east cutter machine was not properly guarded in that there were openings in the rear and in the front at the point of operation from the rotating cog thereby exposing employees to a caught-in hazard.    b.      In the new oxidizer room on the Cutter platform where barrels which were closed head are cut and made into open head barrels, the cutter machine located on the northside of the platform had 2 1/4 and 2 1/2 inch openings which were not guarded from accidental contact thereby exposing employees to a caught-in hazard.    c.       In the new oxidizer room on the Cutter platform where barrels which were closed head are cut and made into open head barrels, the cutter machine on the West side of the platform had opening which were not properly guarded from employees accidently getting caught-in the rotating cogs thereby exposing employees to a caught-in hazard.    d.      In the new oxidizer room on the cutter platform where barrels which were closed head are cut and made into open head barrels, the cutter machine in the middle of the platform had unguarded openings where the cog rotated where employees could become caught-in the rotating part thereby exposing employees to a contact hazard.    e.      In the new oxidizer room on the cutter platform where barrels which were closed head are cut and made into open head barrels, the cutter machine on the north east side of the platform had openings in the front and rear of the machine with the cog exposed and employees were exposed to a caught-in hazard.    f.      In the new oxidizer room on the cutter platform where barrels which were closed head are cut and made into open head barrels, the cutter macine on the south side of the platform had unguarded openings in the front and back of the cutter where the cog rotating in the machine exposed employees to a caught-in hazard.    g.      In the open head platform area where steel barrels are placed in the East Body Roller which reforms the steel drum, the side cover guard was not in place when employees were observed operating the machine thereby exposing employees to a caught-in hazard.

29 CFR 1910.219(e)(1)(i):     Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section.    a.     On the phosforic Line beside the stairway to the north stairway, the belt and pulley were not provided with a guard which fully encloses the exposed belts and pulleys to the Sephon Pump thereby exposing employees to a caught in hazard.    b.     On the phosiforic line, there was no guard installed to the rear portion of the belt and pulley of the Vaccuum Tank Motor of the progressive rinser thereby exposing employees to a contact/caught-in hazard.    c.      On the north end of the chain converor Oxidizer Stack Pad, there was an unguarded belt and pulley beside the conveyor with no guarding on the back side of the guard to protect employees from accidental contact with a unguarded belt and pulley thereby exposing employees to a caught-in/contact hazard.     d.      In the area of the 80 foot oven, the belt and pulley to the north blower motor was not guarded from accidental contact thererby exposing employees to a contact/caught-in hazard.    e.      In the area of the 80 foot oven, the belt and pulley to the south blower motor was not guarded from accidental contact thereby exposing employees to a contact/caught-in hazard.    f.       In the Closed Head Finish Side, on the west side of the closed head blaster(SW Corner) there  was  a unguarded belt and pulley at the bottom of the machine thereby exposing employees to a contact/caught-in hazard.

In the cover department, the energized 480 volt electrical disconnect just east of the womens restroom was not marked with the equipment the disconnect was connected to thereby exposing employees to a contact hazard.    b.     In the new room next the caustic shaker, the energized 480 volt electrical disconnect was not marked with the equipment it operated thereby exposing employees to a contact hazard.    c.
