29 CFR 1910.420(a)(2): The employer did not provide at the dive location a safety practices manual, including a copy of the OSHA standard 1910 Subpart T, Commercial Diving Operations; and including safety procedures and checklists for diving operations, assignments and responsibilities of the dive team members, equipment procedures and checklists, emergency procedures for equipment failure, adverse environmental conditions, and medical illness and injury: a) When engaged in commercial diving operations, the employer did not have at the jobsite a Dive Plan. Employees had been doing supplied-air diving for two weeks at the jobsite; an apartment building on concrete pilings over the East River. During dives the divers penetrated 200 feet (horizontally) under the building. Location: 30 Waterside Plaza, New York, N.Y. 10010 On or about 4/25/2012 NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
