29 CFR 1910.22(a)(2):     Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition:    On or about August 14, 2012 and times prior thereto, employees of Allen's, Inc.  shoveled produce waste from floors and performed quality checks on vegetable canning lines throughout the Country Plant facility located at 14961 Readings Road in Siloam Springs, Arkansas.  Floors were observed with water accumulation in the following instances:    a. Floors of the production area where wet processes were used with machinery had water accumulation, exposing employees performing housekeeping tasks to slipping hazards.    b. The flooring of the hydrostat cooking room had water accumulation approximately greater than 1 inch from the ground, exposing employees to slipping hazards.

29 CFR 1910.23(a)(8):     Every floor hole into which persons can accidentally walk were not guarded:    On or about August 14, 2012 and times prior thereto, employees of Allen's, Inc. performed quality checks on vegetable canning lines throughout the production area of the Country Plant facility located at 14961 Readings Road in Siloam Springs, Arkansas.  Drains had been installed on the flooring in the production area to capture water from wet processes such as but not limited to blanching, and soak tanks.  Drain guards had been removed from the installed drain near the green bean cutters and the bean canning line, creating a hole in the flooring 6 inches deep and exposing employees to tripping hazards.

Unguarded pulleys were observed in the following instances:    a. An employee was observed performing quality control on the green bean line, removing green beans from over filled cans.  An unguarded cable and two pulleys were observed under the conveyor line transporting green beans, exposing the employee to caught-in hazards.    b. An employee was observed performing quality control on cans of potatoes by removing potatoes from over filled cans.  An unguarded belt and pulley was observed under the conveyor line transporting the cans of potatoes, exposing the employee to caught-in hazards.
