29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems: An employee conducting excavation work on an approximately 60 foot by 70 foot excations which was approximately 11 feet deep was exposed to struck by hazards, and the employer did not initiate and maintain an accident prevention program to have daily inspections of the excavation, the adjacent areas, and protective system in accordance with the OSHA regulations.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: An employee was exposed to fall hazards while using an extension ladder which did not extend at least 3 feet over the point of landing at the top of the trench. The employer did not provide a training program which enabled each employee to recognize hazards and the correct procedures to minimize hazards with the set up of an extension ladder.
