29 CFR 1910.1200(f)(5)(ii):  Appropriate hazard warnings, or alternatively, words, pictures, symbols, or combination thereof, which provided at least general information regarding the hazards of the chemicals was not attached to hazardous chemicals:     a) In the Laundry room, on or about September 5, 2012 -  The spray bottle of Morning Mist, which contained ethyl alcohol and didocyl dimethyl ammonium chloride, was not labeled with hazardous warnings such as corrosive.

29 CFR 1910.1200(h)(3)(iv):  The details of the hazard communication program developed by the employer, did not inlcude an explanation of the labeling system and the material safety data sheet, and how employees can obtain and use the appropriate hazard information:    a) In the Laundry room, on or about September 5, 2012 -  The maintenance employees were not trained on the details of the hazard communication program including an explanation of the material safety data sheets and where the copies of the material safety data sheets were located.     Maintenance employees were responsible for changing out the empty containers such as but not limited to the following laundry detergents:  NDT Oxy-Bleach, which contained hydrogen peroxide; laundry bleach, which contained 35% hydrogen peroxide; Power Jolt, which contained sodium hydroxide; and Citri-D, which contained sodium hydroxide and 2-butoxyethanol.
