29 CFR 1926.501(b)(13): Where the employer was engaged in residential construction activities 6 feet (1.8m) or more above lower levels employees were not protected from falling by guardrail systems, safety net systems, or personal fall arrest system:    a) On or about July 30, 2012, employees were exposed to falls excess of 6 feet while installing felt and shingles on a steep pitched roof. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:    a) On or about July 30, 2012, employees were exposed to falls excess of 6 feet while using a extension ladder to gain access to the roof that did not extend 3 feet above the landing nor was it secured.
