The DoAll vertical band saw bottom idler wheel and unused portion of the saw blade was not guarded.  b. The Marvel vertical bandsaw drive wheel and unused portion of the saw blade was not guarded.  c. The vertical boring mill rotating chuck and tooling was not guarded.  d. The Delta vertical drill press rotating chuck and tooling was not guarded.  e. The Kalamazoo horizontal band saw had four inches of the unused portion of the saw blade that was not guarded.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    On 8/15/2012, in the maintenance shop at the radial arm drill press and on the floor was an unsecured electrical receptacle junction box with knockouts that was attached to an electrical flexible cord.     NOTE: Knockout boxes were designed to be secured to a fixed/permanent structure. When knockout boxes are used on the ends of flexible cords, employees gripping the box to attach or disconnect a plug may dislodge the circular scored metal portion in the box wall causing it to come in contact with the hot terminal. The result may lead to an electric shock.
