29 CFR 1926.451(g)(1)(iii): Each employee on a crawling board (chicken ladder) shall be protected by a personal fall arrest system, a guardrail system (with minimum 200 pound toprail capacity), or by a three-fourth inch (1.9 cm) diameter grabline or equivalent handhold securely fastened beside each crawling board;    a.  On or about August 10, 2012 - at the above addressed jobsite, an employee was conducting roofing operations from a crawling board (chicken ladder) approximately fourteen (14) feet above the ground.  The employee was not protected from falls, thereby exposing the employee to a fall hazard.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     a.  On or about August 10, 2012 - at the above addressed jobsite, employees performing roofing operations approximately fourteen (14) feet above the ground were not protected from falls, thereby exposing the employees to fall hazards.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.    a.  On or about August 10, 2012 - at the above addressed jobsite, the fiberglass portable ladder used to access the roof did not extend at least three (3) feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.

On or about August 10, 2012 - at the above addressed jobsite, the fiberglass non-self-supporting ladder was not being used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder, thereby exposing the employees to fall hazards.    b.  On or about August 10, 2012 - at the above addressed jobsite, the aluminum non-self-supporting ladder was not being used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder, thereby exposing the employees to fall hazards.
