29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Lamjen, Inc. / 2254 East 30th Street, Erie, PA: On or about August 16, 2012 the jib hoist at Okuma LB 400M a temporary knockout style receptacle was used to splice an electric cable that controls the jib hoist in that area. The box was used in a portable application and not utilized in accordance with its listing and labeling in that it was not designed for portable applications and was to be surface mounted.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Lamjen, Inc. / 2254 East 30th Street, Erie, PA: On or about August 16, 2012 machines that include, but are not limited to an Okuma 400M was being operated without a side panel in place. The side panel was removed to allow employees to manually operate a sensor to hold the part inside the machine in place. The sensor is near the rotating parts of the machine.
