29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling. On or about August 15, 2012 and times prior thereto, employees were exposed to the hazards of fire and electrical shock where a shop made extension cord located on the welding table in the die repair area. The box containing the recepticles is listed an labeled to be fastened to a surface and not used as part of a portable extension cord.

29 CFR 1910.1200(h)(3)(ii): Employee training did not include the physical and health hazards of the chemicals in the work area: On or about August 15, 2012 and times prior thereto in the paint touch-up area, employees did not receive specific training on the hazards and steps to take to limit exposure to paint vapors that contain organic solvents such as but not limited to acetone, n-butylacetate and 2-methoxy-1-methylethyl acetate.
