29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: Maintenance shop: On or about August 9, 2012 and times prior thereto, the Wilton 16 in drill had no guard at the point of operation. This exposed employees to laceration/amputation hazards to the hands.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): Bunk House: On or about August 9, 2012 and times prior thereto, the cover on the Rheem water heater was not installed. This exposed employees to electric shock hazard of up to 240 volts from accidental contact with energized parts.
