29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    JOBSITE:  The employees, exposed to the hazard of falling while installing roof shingles on the low pitched roof (2 in 12 pitch) were not provided with information and training to control or minimize such hazards.

29 CFR 1926.1053(b)(5)(i):  Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and top support):    JOBSITE:  The horizontal distance from the vertical support to the base of the 32 foot aluminum extension ladder which was used by the employees to access the roof of a building to perform work was more than one fourth (1/4) of the working length of the ladder.

29 CFR 1926.1060(a):  The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  Note:  29 CFR 1926.21(b)(2) may be cited as an additional violation to support this requirement.    JOBSITE:  The employees, using an aluminum extension ladder to have access to an upper working level were not provided with information and training regarding the safe use and handling of the ladder.

29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50 feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    JOBSITE:  The employees, engaged in roofing activities on the low sloped 2 in 12 pitched roof were not proteced from the hazard of falling approximately 23 feet to the ground by use of appropriate types of guardrail and/or personal fall arrest systems or other safe means.
