29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, and enclosures was not permanent and continuous: At the back door exterior of house BB Lot 2: On or about August 15, 2012, the grounding pin was missing on an extension cord that supplied power to a 110 VAC Makita jack hammer. The jack hammer was used to breakup a concrete pad.

29 CFR 1926.600(a)(3)(i): Bulldozer and scraper blades, end-loader buckets, dump bodies, and similar equipment, were not either fully lowered or blocked when being repaired or when not in use: Near the north side of house 104, BB1 L1: On or about August 15, 2012, and at times prior thereto, a Caterpillar 420 E backhoe's stabilizers held the rear tires off the ground. The operator was not within 25 feet, and the backhoe was turned off.
