29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: a) Electrical Panel 1-A, Near Print Shop Entrance, on or about 8/16/2012; Missing 5 breaker switches, 1 knock-out enclosure at the bottom and the panel cover was not secure, exposing employees to shock and burn hazards. b) Electrical Panel 1-B, Press Room next to stand-up desk, on or about 8/16/2012; 15 knock-out enclosures at the panel, exposing employees to shock and burn hazards while working at the desk. c) Electrical Panel 2-A, Bindering Area, on or about 8/16/2012; Missing 1 lower right side breaker switch, 3 knock-out enclosures at both sides and top, and the panel cover was not secure, missing 4 of 6 screws, exposing employees to shock and burn hazards. d) Electrical Panel 3-A, Bindering Area, on or about 8/16/2012; Missing upper left side breaker switch labeled "Warehouse Breaker" exposing employees to shock and burn hazards. e) Conduit boxes next to Panel 3-A, Bindering Area, on or about 8/16/2012; Missing covers on two 90 degree elbow fittings exposing employees to shock and burn hazards.

29 CFR 1910.1200(f)(10): The employer did not ensure that workplace labels or other forms of warnings were prominently displayed on the container or readily available in the work area throughout each work shift. a) Press Room, on or about 8/16/2012; three bottles allegedly containing VWM Press Wash were not labeled with the container contents or other forms of warning. One bottle on the Press Room table and two bottles on the Heidelberg Quickmaster # 99570. b) Press Room stand-up desk, on or about 8/16/2012; a spray bottle allegedly containing Windex was not labeled with the container contents or other forms of warning.
