29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval:    On or about 8/13/12 at 125 Union Hill Road Canton, GA; an employee performing welding operations was exposed to fall hazards of 20' to the lower level where a trash bin was placed on the forks of an RT forklift with the employee inside and elevated to the working height.
