29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      On or about August 15, 2012, and at times prior thereto at the facility in three instances the employer allowed employees to operate various machines without a guard provided:    1: In the machine shop a CNC lathe was used without an interlock device provided to prevent employee contacting the chuck when the door is opened while in operation.      2: At the press brake area a press brake was used without guard provided.    3: At the shear area a guard was provided for the shear however the openings of the guard were big enough to allow employees hands or fingers to enter the point-of-operation while in operation.      Employees were exposed to an amputation caused by the unguarded point-of operation.

Employees were exposed to flying fragments in the event of an exploding wheel.

The employees were exposed to skin and eye injury.
