29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. The employer does not protect employees from falling when working more than 10 feet above a lower level on a scaffold. This violation was observed on or about August 14, 2012, on a pump jack scaffold at the job site where employees were exposed to a 30 foot fall hazard when working from the scaffold.

29 CFR 1926.452(j)(1): Pump jack brackets, braces, and accessories were not fabricated from metal plates and angles. Each pump jack bracket did not have two positive gripping mechanisms to prevent any failure or slippage. The employer does not ensure that pump jack scaffold braces are fabricated from metal plates. This violation was observed on or about August 14, 2012, on the west side of the structure where an employee was exposed to a fall hazard while working from a pump jack scaffold that had plywood bracing.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. The employer does not provide bracing to the bottom of pump jack scaffolds. This violation was observed on or about August 14, 2012, on the west side of the structure where an employee was exposed to a fall hazard while working from a pump jack scaffold did not have bracing at the bottom of the scaffold, below the working surface
