On or about 08/16/12, at the site of 148-18 3rd Ave. Whitestone, NY Employees working underneath of scaffolds were in danger of head injury from falling objects such as, but not limited to tools and buckets of stucco from other employees who working on above section of the scaffolds. Protective helmets were not provided. ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked: A. On or about 08/16/12, scaffolds at 148-18 3rd Ave. Whitestone, NY Employees working on the fabricated frame scaffolds in the front of the houses were exposed to the hazard of falling approximately 13 ft. and 15 ft. to the ground, respectively. The working platforms were installed with only two or three scaffold planks. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access: A). On or about 08/16/12, at 148-18 3rd Ave. Whitestone, NY Employees were ascending/descending scaffolds by climbing the frame and were in danger of falling approximately 15 ft to the ground. An access ladder or equivalent safe devices were not provided. ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.

Guardrails or personal fall arrest systems were not provided. ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
