29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation: The employer does not provide baseplates for scaffolds legs. This violation was observed on or about August 14, 2012, on the northwest side of the structure where employees were exposed to a 14.5 foot fall hazard while using a scaffold that lacked baseplates.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. The employer does not provide guardrails for scaffolds for employees working 10 feet or more above a lower level. This violation was observed on or about August 14, 2012, on a scaffold at the job site where employees were exposed to 14.5 foot fall hazard while working from a scaffold that did not have guardrails.
