On or about August 16, 2012, the operator was not protected from the rotating tool of the machine when the guard doors remained open during operation. b) Mori Seiki MV45 Vertical Milling Center, production floor. On or about August 16, 2012, the operator and other employees in the work area were not protected from flying metal chips from the milling process when the guard remained open during operation.

On or about August 16, 2012, the work rest on the pedestal grinder was one-fourth of an inch from the grinding wheel.
