29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when:    a)  At Excavation in Pine Hill, AL: On or about August 6, 2012 and at times prior, the employer exposed employees to engulfment hazards, in that cave in protection was not provided to employees while boring under HWY 5 to install water transmission lines.    The Cardinal Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA inspection number 312697352, citation number 1, item number 4 and was affirmed as a final order on April 23, 2009, with respect to a workplace located at State Hwy 181 & Camberwell Dr.
