29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions:    a) Job site, 2628 East Carson Street, Pittsburgh, PA: On or about August 15, 2012, during construction activities, the employer did not ensure that an excavation/trench competent person inspected the excavation employees were working in for hazardous work conditions. Employees were working in an excavation determined to be approximately 15 feet in depth with an inadequate cave in protection system, exposing workers to excavation wall collapses.

Employees were working in an excavation determined to be approximately 15 feet in depth with inadequate cave in protection, exposing workers to being caught in an excavation wall collapse.
