29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    (a) On or about November 20, 2012, and at times prior, for the employees who climb a fixed platform approximately five feet and 10 1/2 inches from the floor in order to add chemicals (Clarington and muriatic acid) into a large container with Nalco 7607 Plus in the production area of Pure Pulp Products, Inc. located at 25329 U.S. Highway 281 in Plankinton, South Dakota.    Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.
