29 CFR 1926.701(b): Reinforcing steel. All protruding reinforcing steel, onto and into which employees could fall, shall be guarded to eliminate the hazard of impalement. (a) Assisted Living Facility - On or about August 15, 2012 the employer had not installed caps on rebar protruding from the floor of a front door passageway exposing employees to an impalement hazard.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. a) Assisted Living Building - On or about August 22, 2012 employees were exposed to the hazard of tripping or stepping into a 7 inch deep, 6 inch wide and 8 foot long floor hole running across a door in the back of the facility. Employees were also exposed to tripping or stepping into another 5 inch wide, 6 inch deep and 4 foot long floor hole running acrpss another door in the back of the facility.
