29 CFR 1926.300(b)(5): "Exposure of blades." When the periphery of the blades of a fan is less than 7 feet (2.128 m) above the floor or working level, the blades shall be guarded. The guard shall have openings no larger than 1/2 inch (1.27 cm). (a) Job Site - On or about August 16, 2012 a fabricated box fan use to circulate air in the work area had a guard with opening 4 inches by 2 inches, thereby, exposing employees to the hazard of being struck by the blades.

Hard service flexible cords No. 12 or larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced.
