29 CFR 1910.1030(c)(1)(iv)(B): The review and update of the exposure control plan did not document annual consideration and implementation of appropriate commercially available and effective safer medical devices designed to eliminate or minimize occupational exposure:    (a) On or about August 15, 2012, and at times prior, for employees throughout the facility with potential occupational exposure to bloodborne pathogens falling under the Exposure Control Plan, who may use Kendall Monoject Insulin Syringes, Tuberculin Safety Syringes, Vacutainer Brand Safety-Lok Blood Collection sets, BD Vacutainer Eclipse Blood Collection Needles, BD Ultra-Fine Pen Needles, and/or Assure Haemolance Plus Safety Lancets, where the annual review was not documented at 740 East Lake Street in Lake Andes, South Dakota.    Note:  This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 312401425, Citation 1, Item 1A, and was affirmed as a final order on 11/19/2008, with respect to a workplace located at 740 East Lake Street, Lake Andes, South Dakota.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:    (a) On or about August 15, 2012, and at times prior, an electrical panel in the kitchen of Lake Andes Health Care Center, located at 740 East Lake Street in Lake Andes, South Dakota, was blocked by a refrigerator that was placed approximately six inches (6") in front of the panel, where a minimum distance of 36 inches should be allowed for access into the electrical panel.
