29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    On or about August 10, 2012 at the back parking area and building work area located at 4367 Old McDonough Road Conley, GA;    Where employee's were replacing an air fliter, troubleshooting and replacing a hydraulic line on the yard truck (dog), no lockout or tagout devices were not used to de-energize the vehicle.

29 CFR 1910.178(p)(1): Where powered industrial trucks were found to be in need of repair, defective, or in any way unsafe, the trucks were not taken out of service until they had been restored to safe operating condition    On or about August 13, 2012 at 4367 Old McDonough Road Conley, GA;    The employees operated the Ottawa yard truck and Komatsu powered industrial truck on uneven ground in the parking area with deficiencies such as but not limited:     Yard Truck (Ottawa):   1. Leaking hydraulic line for the cab lifting and lowering device.  2. Broken cab safety prop bracket.  3. Broken and missing dashboard indicators, lights and controls.  4. Broken cab safety prop cable.  Komatsu (Forklift)  1. Hand brake not working to prevent the equipment from moving.  2. Electrical horn not working to warn other employees walking or working in the area.  3. Broken right and non-secured left propane tank brackets.

29 CFR 1910.134(h)(1): Respirators were not cleaned and disinfected using the procedures in Appendix B-2 of 29 CFR 1910.134 or equivalent procedures recommended by the respirator manufacturer:    On or about August 13, 2012 at 4367 Old McDonough Road Conley, GA;    The scott AV-2000 full face tight fitting respirators use by the employees when entering confined space of tankers to inspect and clean the tankers had dust practicals outside and inside the respirators and were not properly cleaned or disinfected before being stored or used.
